6-9" hook and spatula: The hook and spatula are unique instruments in laparoscopic surgery because they are the only major dissecting instruments held in a pen-like grip. This affords superior and precise control, enabling to surgeon to carry out fine dissection using monopolar electrosurgery. I wanted to extend the same benefit to open surgery and designed a 6-9" hook and spatula. These new instruments are extremely useful for dissection using monopolar electrocautery when the field of surgery is in the depths of the body cavity, e.g. Whipple's pancreatoduodenctomy and other pancreatic surgery or deep in the pelvis cavity, e.g. anterior resection and other rectal surgeries.
The other instrument that has been improved upon is the Maryland. The standard Maryland is the commonest dissecting instrument used by the surgeon for day-to-day surgeries like lap cholecystectomy and TEP hernioplasty. The disadvantage was the (relatively) long jaw and joint area which was uninsulated and there was always a fear of unintentional direct coupling injury when using monopolar cautery with it, especially in narrow regions like dissecting in the Calot's triangle or smaller spaces like the extraperitoneal space during TEP hernioplasty. I modified the standard Maryland into this device with a much small jaws and to reduce the joint area, the lower jaw was designed to be fixed. Thus the uninsulated area is now a third of what it was originally.

Dr Arvind Ganpule is a postgraduate guide and examiner for urology trainees in India. He has 140 pubmed indexed publications to his credit in reputed peer reviewed National and International Journals. In addition, he has authored 23 book chapters and edited three books. He was a lead investigator in the largest epidemiological study in India, investigating the natural history of BPH in western India. He has innovated a novel low cost retrieval bag in laparoscopy.(known as the Nadiad bag). He was involved with the team at MPUH, Nadiad India in developing the microperc system with Polydiagnost GmBH. He has two patents filed with Indian patent office.
